Florida Sports Physicals: The EL2 Form and What We Check
What the FHSAA EL2 physical involves, how long it stays valid, and how to prepare so your athlete is cleared on time.
Every student athlete in Florida needs a completed preparticipation physical before tryouts. Knowing what is checked, and what commonly delays clearance, keeps your athlete on the field.
What the exam covers
- Detailed personal and family cardiac history.
- Heart and lung examination, blood pressure, and pulse.
- Vision screening and eye examination.
- Musculoskeletal assessment of joints, strength, and flexibility.
- Review of prior concussions, injuries, asthma, and current medications.
Paperwork and timing
- The FHSAA EL2 preparticipation physical form must be signed by a licensed provider.
- Physicals are valid for 365 days from the exam date.
- The EL3 consent and release form is completed by parents.
- Schedule several weeks before tryouts so any follow-up testing fits in.
Why cardiac history matters
The most important part of the visit is often the questionnaire. Fainting during exercise, chest pain with activity, unexplained racing heartbeat, or a family history of sudden death before age 50 all prompt further evaluation before clearance.
How to prepare
- Bring the school's forms already filled out on the family section.
- Bring inhalers, braces, or orthotics your athlete uses.
- Ask family members about heart conditions and sudden deaths before the visit.
- Wear or bring athletic clothing for the movement portion.
