Privacy Policy

1. Scope

This Privacy Policy applies to information collected through the Divaker Pediatrics marketing website. It does not replace the practice's HIPAA Notice of Privacy Practices, which governs Protected Health Information (PHI) held by the practice and is provided at the office.

2. What we collect

  • Information you submit voluntarily — name, email address, phone number, and the contents of any message you send through website forms (e.g. the data-rights request form).
  • Automatically collected — standard log data such as IP address, browser type, referring page, and pages visited. This is used for security, spam prevention, and basic site operation.
  • Cookies — see our Cookie Policy. The site currently uses only strictly-necessary cookies plus a cookie-consent preference cookie.

Do not submit private medical information — symptoms, diagnoses, medications, or treatment history — through this website. For medical questions call the office at 352-383-8386 or use the secure patient portal.

3. How we use it

  • To respond to inquiries and privacy requests.
  • To operate, secure, and improve the website.
  • To comply with legal obligations.

We do not sell your personal information. We do not currently run advertising pixels, session-replay, or third-party analytics on this website.

4. Sharing

We share information only with service providers required to operate the website (hosting, database, email delivery) under contractual confidentiality obligations, and where required by law. Third-party booking (healow) and the patient portal (eClinicalWorks) are governed by their own privacy notices and, where PHI is involved, Business Associate Agreements with the practice.

5. Your choices

You can request access, correction, deletion, portability, or opt-out via the Data / Privacy Request form. We verify identity before acting on requests involving personal information.

6. Security

The website is served over HTTPS. Please see our Security & Trust page. No method of transmission over the internet is 100% secure.

7. Children's privacy

Because Divaker Pediatrics serves children, parents and guardians interact with this site on behalf of minors. The website is not directed at children under 13 for the purpose of collecting personal information from them directly.
