1. Notice of Privacy Practices
Divaker Pediatrics maintains a HIPAA Notice of Privacy Practices that describes how Protected Health Information (PHI) about your child may be used and disclosed, and how you can get access to this information. A copy is provided at your first office visit and is available upon request by calling 352-383-8386.
2. This website is not a secure channel for PHI
Please do not submit symptoms, diagnoses, medications, lab results, treatment history, or any other private health information through this website's forms or email. Website submissions are not encrypted end-to-end for PHI transmission.
For anything involving your child's health, use one of these:

- Request an appointment through healow.
3. Vendors and BAAs
Where third-party vendors create, receive, maintain, or transmit PHI on behalf of the practice, a Business Associate Agreement (BAA) is required. Confirming BAA status with each vendor is an operational responsibility of the practice and is tracked in the internal vendor inventory.
4. Your rights
Your rights under HIPAA — including access, amendment, accounting of disclosures, restrictions, and confidential communications — are described in the practice's Notice of Privacy Practices.
