Back-to-School Checkups in Florida: What Your Child Needs
Required Florida school health and immunization forms, what happens at the visit, and why summer is the best time to book.

Florida schools require specific documentation before a child can start, and summer appointments fill fast. Here is exactly what to line up.
What Florida requires
- Form DH 3040, the school entry health examination, within 12 months of enrollment.
- Form DH 680, the certification of immunization.
- Additional immunization requirements at seventh grade, including Tdap.
- Sports participation forms for student athletes.
What happens at the visit
- Growth measurements, blood pressure, and a full physical exam.
- Vision and hearing screening.
- Review and catch-up of any due immunizations.
- Discussion of sleep, nutrition, screen time, mood, and school readiness.
- Completion and signing of the required forms.
Book early
July and early August are our busiest weeks. Scheduling in May or June means better appointment times and no last-minute scramble before the first day.
Bring with you
- Any forms the school gave you.
- Your child's immunization record if care was received elsewhere.
- A list of current medications and any specialist notes.
- Your questions, written down.
